Dental coverage is divided into three types:

Type A - Preventive: In-network services are paid at 100% with no deductible.

Type B - Basic Restorative: In-network services are paid at 80% after the $50 annual individual deductible.

Type C - Major Restorative: In-network services are paid at 50% after the $50 annual individual deductible.

The annual maximum dental benefit is $1,000 per person.
